Sodalis Living is committed to protecting the privacy and confidentiality of every resident’s health information. This policy outlines how we collect, use, and protect protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and its implementing regulations.

We’re continuously working on improvements and updates to ensure the accessibility of our corporate and community websites.

Who This Policy Covers

This policy applies to Sodalis Living and its affiliated communities, including all team members, contractors, and business associates who handle PHI on behalf of Sodalis Living.

Moreover, we may collaborate with third-party service providers who use cookies to track site activity on our behalf. These entities solely use cookies to provide us with information about site traffic.

What Information We Collect

As part of providing assisted living, memory care, and related services, we may collect and maintain the following types of PHI:

  • Personal identifiers, including name, address, date of birth, and Social Security number
  • Medical history, diagnoses, and current health conditions
  • Medication records and treatment plans
  • Insurance and billing information
  • Records from referring physicians, specialists, and other healthcare providers

How We Use and Disclose PHI

Sodalis Living uses PHI only as permitted or required by HIPAA. This includes:

  • Providing and coordinating care for residents
  • Communicating with physicians, specialists, and other authorized healthcare providers involved in a resident’s care
  • Processing billing and insurance claims
  • Complying with applicable federal, state, and local laws and regulations
  • Supporting internal operations such as quality reviews and compliance audits

We do not sell PHI. We do not use PHI for marketing purposes without written authorization from the resident or their authorized representative.

Your Rights as a Resident or Authorized Representative

Under HIPAA, residents and their authorized representatives have the right to:
  • Access and request copies of health records
  • Request corrections to inaccurate or incomplete records
  • Request restrictions on certain uses or disclosures of PHI
  • Receive an accounting of disclosures made without authorization
  • Request that communications be conducted through a specific method or location
  • Receive a paper copy of this notice upon request

How We Protect Your Information

Sodalis Living maintains physical, administrative, and technical safeguards to protect PHI from unauthorized access, use, or disclosure. Team members receive training on HIPAA requirements and are required to handle all resident information with care and discretion.

How to Submit a Request or File a Complaint

To exercise any of the rights described above, or to report a concern about the privacy of your health information, please contact your community’s Executive Director or reach out to Sodalis Living’s Privacy Officer directly.

You also have the right to file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights at hhs.gov/ocr. Sodalis Living will not retaliate against any individual for filing a complaint.
